120–130mmHg
General HTN: target SBP 120–130 mmHg (SPRINT/2023 ESH) — Current SBP 150 is OUTSIDE target
How to use it
- 1
Select patient group. Indication-specific BP targets differ by guideline. Individualise for frailty, tolerance, and polypharmacy.
- 2
Enter current SBP. Use validated sphygmomanometer; ≥2 readings, seated, after 5 min rest.
- 3
Compare to target. If outside target: adjust medications. Consider home BP monitoring (typically 5/7 mmHg lower than clinic readings).
When to use it
- Medication review in hypertension
- Ensures targets are specific to comorbidities. General target ≠ DM target ≠ CKD target.
- Discharge planning
- Ensure discharge BP medications are dosed to achieve target within 1–4 weeks.
- Polypharmacy assessment
- Frail elderly patients: BP targets may be relaxed to prevent falls/orthostatic hypotension.
