ABCD² Score calculator
Result
1/ 7
Low (2-day stroke risk ~1%)
How to use it
- 1
Record age. ≥60 years = 1 point.
- 2
BP on presentation. SBP ≥140 OR DBP ≥90 = 1 point.
- 3
Clinical features. Unilateral weakness = 2 points; speech disturbance without weakness = 1 point.
When to use it
- TIA triage in ED
- Score ≥4 warrants same-day specialist neurovascular assessment and imaging (DWI MRI > CT).
- Deciding admission vs. rapid outpatient pathway
- NICE recommends next-day specialist assessment for all TIA regardless of ABCD². Score helps risk-stratify urgency.
- Note limitations
- ABCD² has poor positive predictive value in isolation. DWI MRI and carotid imaging are more informative.
