Not at risk (<3) — rescreen weekly
How to use it
- 1
If any initial screen positive. Check all 3 initial criteria (BMI, weight loss, reduced intake, severe illness) — if all negative and patient not severely ill: rescreened weekly.
- 2
Full score. Disease score + nutrition score + 1 if age ≥70.
- 3
Score ≥3. Dietitian referral + nutritional support within 24–48h.
When to use it
- ESPEN endorsed hospital tool
- NRS-2002 (Kondrup et al.) validated in clinical trials as predictor of benefit from nutritional support.
- ICU nutritional risk
- Combined with acute severity: ICU patients with NRS ≥3 benefit from early enteral feeding.
- Gastrointestinal surgery
- Pre-op NRS ≥3: ESPEN recommends 7–14 days nutritional support before elective GI surgery.
A reference for clinicians and students, not medical advice. A score supports a decision; it does not make one. Check every result against the patient in front of you and your local protocol.
