Corrected Sodium (Hyperglycaemia)
True sodium adjusted for glucose-induced dilution.

Result
137.8mmol/L
Corrected sodium normal
For each 5.6 mmol/L glucose above 5.6, Na is diluted by ~2 mmol/L.
How to use it
- 1
Measured Na may be spuriously low in DKA/HHS. Glucose draws water from cells → dilutes extracellular Na.
- 2
Correction factor: 0.4 mmol Na per 1 mmol/L glucose above 5.6. Some use 0.3 (Katz) or 0.24 (Hillier) — 0.4 is most widely used.
- 3
Guides fluid choice. High corrected Na in HHS → use 0.45% saline initially.
When to use it
- DKA and HHS
- Measured Na low in DKA: corrected Na guides saline tonicity choice.
- HHS (hyperosmolar)
- Corrected Na + glucose together estimate effective osmolality: 2×Na + glucose.
- Monitoring insulin therapy
- As glucose falls, measured Na should rise proportionally.
