Richmond Agitation-Sedation Scale (RASS) calculator
Result
RASS 0
Target for most ICU patients — light sedation preferred
How to use it
- 1
Observe 30 seconds. Spontaneous behaviour first. Then call patient's name. Then physical stimulation.
- 2
Target RASS 0 to −1. PADIS 2018: lightest sedation that tolerates ventilator and keeps patient safe.
- 3
Pair with CAM-ICU. RASS −2 to 0: perform CAM-ICU delirium check.
When to use it
- ICU sedation protocol
- Target RASS −1 to 0 unless specific indication for deeper sedation.
- Analgesia-first (A1C)
- Pain (NRS/CPOT) → analgesia → reassess RASS → consider sedation only if needed.
- Daily SAT/SBT
- RASS ≥ −2: spontaneous awakening trial eligible. RASS −3 to −5: not yet.
