Limiting the psychological impact of hospitalization for allogeneic transplantation in AYAs in adult department: Guidelines from the SFGM-TC
Abstract
Allogeneic hematopoietic stem cell transplantation (HSCT) is a major therapeutic step in the care of adolescents and young adults (AYA) with hematologic cancers. However, this prolonged hospitalization, marked by strict isolation, may have significant psychosocial repercussions for a population in the midst of identity construction. This article, developed from a harmonization workshop of the SFGM-TC, seeks to formulate recommendations for adapting hospitalization conditions to better suit AYAs. A survey conducted across 33 transplant centers reveals variability in hospital practices regarding isolation, visitation policies, dietary restrictions, and access to supportive care. Based on the survey results, a literature review, and interviews with healthcare professionals, concrete solutions are proposed: anticipation of hospitalization, individualization of the framework, limitation of isolation outside of aplasia, maintenance of social ties, spatial planning, and access to appropriate supportive care. The objective is to improve the experience of hospitalization, promote the therapeutic alliance, and limit the risk of psychological after-effects for this vulnerable population.
