Very low risk — median survival ~8.8 yrs
How to use it
- 1
Requires cytogenetics, BM biopsy, and CBC. Blasts from BM aspirate + cytogenetics from karyotype or FISH.
- 2
IPSS-R replaces IPSS (2012). Better stratification, especially for low-risk disease.
- 3
Risk group guides treatment. Very low/Low: supportive care (EPO, lenalidomide). High/Very high: HMA + alloSCT.
When to use it
- MDS diagnosis and treatment planning
- IPSS-R is the standard risk tool for newly diagnosed MDS (WHO 2022).
- Transplant decision
- Intermediate/High: refer for alloSCT assessment if fit.
- HMA therapy initiation
- Azacitidine: High/Very high IPSS-R — improves OS vs. best supportive care.
