How to use it

Assess overall functional level. Focus on physical activity and self-care capacity over last 24–48h.

ECOG 0–1: standard therapy. Most clinical trial inclusion criteria: ECOG 0–1 (or 0–2 for some regimens).
- 3
ECOG 3–4. Clinical trials typically exclude. Palliative/BSC appropriate.
When to use it
- Chemotherapy eligibility
- ECOG ≥2 is relative contraindication to platinum-based doublets.
- Immunotherapy
- PD-1/PD-L1 inhibitors: ECOG 2 increasingly included in modern trials.
- Prognosis
- ECOG PS is the strongest single predictor of response and survival in solid tumours.
