BISAP Score calculator
Result
0/5
Low risk (~1% mortality)
How to use it
- 1
All 5 criteria at admission. Advantage over Ranson: no 48h wait needed.
- 2
SIRS criteria. Temp <36 or >38°C, HR>90, RR>20, WBC<4 or >12×10⁹ — need ≥2.
- 3
Score ≥3. Organ failure in ~20% of patients; HDU/ICU management.
When to use it
- ED pancreatitis triage
- BISAP faster than Ranson at predicting severe pancreatitis on admission.
- ICU threshold
- BISAP ≥3 + CT showing necrosis → multidisciplinary pancreatitis team.
- CTSI supplement
- BISAP 0–1 can avoid CT in most patients — scan if clinical deterioration.
