頭頸部癌の化学放射線療法における口腔粘膜炎に対する半夏瀉心湯

Abstract
癌治療における口内炎対策として, 口腔ケアをはじめ, 多職種連携によるさまざまな取り組みが行われてきた. また, 粘膜炎の病態として生物学的 phase も知られるようになり, 薬剤がどの phase においてどのように有効かの基礎的裏付けもされるようになってきた. がん支持療法の国際学会である Multinational Association of Supportive Care in Cancer (MASCC) および International Society for Oral Oncology (ISOO) によるガイドラインには, エビデンスレベルとともに推奨・非推奨等が紹介されている. さらに, 日本では厚生労働省による重篤副作用疾患別対応マニュアル (抗がん剤による口内炎) に予防や治療の内容が紹介されており, このマニュアルには治療方法の一つとして半夏瀉心湯が明記されている. 頭頸部癌の放射線治療や化学放射線療法では口内炎対策が不可避であり, 半夏瀉心湯の効果も期待される. これまで小規模の報告が散見される程度であるが, 粘膜炎の軽減を示す報告が大半である. 今後, より高いエビデンスレベルでの検証が求められる.
