ERα Driven Neutrophil Reprogramming Sustains NET Mediated Laminin Remodeling and Late Dormancy Escape in HR+ Breast Cancer
By Zsidisin GN.
Abstract
Late recurrence defines estrogen-receptor-positive breast cancer. Postpartum breast cancer demonstrates that a locally remodeled mammary niche can raise the risk of distant metastasis, with postpartum involution and its associated matrix remodeling and immune influx proposed as the mediator. After five years of endocrine therapy, distant recurrence continues at a near-constant annual rate for at least fifteen further years and scales with original tumor and nodal burden across nearly 63,000 patients in the EBCTCG meta-analysis. This pattern is most consistent with dormant residual tumor cells reactivated intermittently by microenvironmental signals, and it raises a specific question: what tissue mechanism sustains that reactivation probability across decades. This paper proposes that ERα-skewed estrogen signaling drives a self-reinforcing thromboinflammatory loop capable of repeatedly awakening dormant disseminated tumor cells. Estrogen reprograms neutrophils toward a NET-prone, protumoral phenotype. Persistent neutrophil extracellular traps, stabilized by complement and coagulation components, assemble into a scaffold at the laminin-rich basement membrane interface where dormant cells reside. NET-associated proteases remodel laminin, exposing a cryptic epitope that engages integrin α3β1 and activates FAK/ERK/YAP signaling to drive dormant cells back into cycle. YAP then suppresses PER2, and PER2 loss stabilizes ERα, closing a feedback loop that preserves the scaffold-permissive inflammatory state. The loop becomes self-sustaining only when multiple redundant brakes fail together, which is the proposed explanation for the non-plateauing recurrence hazard. The individual steps of this loop are supported to differing degrees, and several remain unbuilt. Their spatial co-localization in human ER+ breast tissue has never been examined. Multiplexed spatial imaging of ER+ tumors stratified by recurrence outcome, targeting NET, complement, coagulation, and ECM scaffold markers at ERα-rich laminin interfaces with adjacent nuclear YAP, is the critical next experiment. Absence of this architecture would falsify the hypothesis in its present form.
