Chinese guideline for the diagnosis and treatment of hospital-acquired pneumonia and ventilator-associated pneumonia in adults (2026 edition): protocol

This document serves as the protocol for the development of the Chinese Guideline for the Diagnosis and Treatment of Hospital-Acquired Pneumonia and Ventilator-Associated Pneumonia in Adults (2026 Edition). Since the publication of the previous edition in 2018, the field of HAP/VAP diagnosis and treatment in China has undergone substantial changes, including updates in prevention concepts and measures, evolution of the pathogen spectrum and antimicrobial resistance patterns, widespread adoption of rapid molecular diagnostic technologies, the advent of novel antimicrobial agents, and growing demand for artificial intelligence applications in clinical practice; an update based on the latest local epidemiologic and evidence-based data is therefore urgently needed. This guideline is jointly initiated by the Infection Group of the Chinese Thoracic Society and the Respiratory Infection Group of the Chinese Association of Chest Physicians, and has been registered with the International Practice Guideline Registry(PREPARE-2026CN795). The guideline working group comprises a steering committee, a writing expert group, a secretariat, and an external review group, covering four major topics: prevention, diagnosis, antimicrobial therapy, and other treatments. Through systematic searches of Chinese and English databases, relevant research evidence will be appropriately included; methodologic quality will be appraised according to the 2011 Oxford Centre for Evidence-Based Medicine Levels of Evidence, and recommendations will be formulated through expert discussions and consensus meetings. The final guideline will be drafted in accordance with the RIGHT (Reporting Items for practice Guidelines in HealThcare) statement and is scheduled for publication in 2026, aiming to enhance the standardization of HAP/VAP diagnosis and treatment in China and to reduce the associated disease burden.

本文件是《中国成人医院获得性肺炎（HAP）与呼吸机相关性肺炎（VAP）诊断和治疗指南（2026年版）》的制订计划书。自上一版指南于2018年发布以来，我国HAP/VAP诊治领域出现了预防理念和措施的更新、病原谱及耐药菌分布的演变、快速分子诊断技术的普及、新型抗菌药物的问世以及人工智能临床应用的需求等诸多变化，亟需基于最新本土流行病学与循证证据进行更新。本指南由中华医学会呼吸病学分会感染学组与中国医师协会呼吸医师分会呼吸系感染学组联合发起，已完成国际实践指南注册。指南工作组由指导委员会、写作专家组、秘书组和外部评审组构成，涵盖预防、诊断、抗菌治疗及其他治疗四大专题。将通过系统检索中英文数据库，恰当采纳研究证据，依据2011版牛津循证医学证据等级进行质量评价，并通过专家讨论以及共识会议形成推荐意见。最终指南将按RIGHT规范撰写，并计划于2026年发布，旨在提升我国HAP/VAP诊疗规范化水平，降低疾病负担。.
