Vancomycin AUC/MIC Target calculator
Result
1698mg/24h
Divide across dosing interval. Target AUC₂₄ 400 mg·h/L
CL = 4.2 L/h. Adjust at steady-state with Bayesian software (e.g., DoseMe, InsightRx).
How to use it
- 1
AUC/MIC 400–600 mg·h/L target. ASHP 2020 guidance: AUC₂₄/MIC 400–600 for MRSA (assuming MIC = 1 mg/L).
- 2
Estimate initial dose. CL vancomycin ≈ CrCl × 0.689. Dose ≈ AUC target × CL.
- 3
Bayesian software preferred. DoseMe, InsightRx, or PrecisePK: two-level Bayesian monitoring gives most accurate AUC.
When to use it
- MRSA bacteraemia and serious infections
- ASHP guideline: AUC-guided dosing reduces nephrotoxicity vs. trough-only monitoring.
- Trough monitoring no longer recommended
- Trough 15–20 mg/L associated with nephrotoxicity without ensuring efficacy — abandoned in 2020 ASHP guideline.
- Renal dose adjustment
- eGFR drives half-life estimate; recalculate after eGFR change.
