Gentamicin Q24h — Hartford nomogram
Check 6–14h post-dose level for HD dosing interval (Hartford nomogram chart).
How to use it
- 1
Use IBW if patient obese. Gentamicin: distribute in lean body mass. Use AdjBW = IBW + 0.4×(ABW−IBW) if very obese.
- 2
Hartford nomogram. Draw 6h and 14h post-dose levels. Point on nomogram chart determines safe interval (q24/q36/q48).
- 3
Traditional TDM. Pre-dose level < 1 mg/L (gentamicin), peak 8–10 mg/L; or OD dosing with Hartford nomogram.
When to use it
- Serious gram-negative infections
- HAP, VAP, febrile neutropenia, pyelonephritis: OD aminoglycoside provides concentration-dependent kill.
- Synergy in endocarditis
- Streptococcal/enterococcal IE: low-dose gentamicin 3mg/kg/day in 3 divided doses (NOT OD dosing).
- Avoid in CKD < 30 mL/min without ID supervision
- Ototoxicity and nephrotoxicity risk — specialist monitoring.
