4.55
Moderate (3.2–5.1)
How to use it
- 1
28-joint assessment. Standard 28 joints: shoulders, elbows, wrists, MCPs, PIPs (10 each side) + knees.
- 2
DAS28-ESR formula. DAS28 = 0.56√TJC + 0.28√SJC + 0.70ln(ESR) + 0.014×GH.
- 3
DAS28-CRP variant. Replace ESR with CRP: DAS28-CRP = 0.56√TJC + 0.28√SJC + 0.36ln(CRP+1) + 0.014×GH + 0.96. Lower threshold for remission.
When to use it
- NICE biologic eligibility
- Active RA: DAS28 > 5.1 on two occasions 1 month apart + DMARD failure = biologic eligible.
- Treat-to-target (T2T)
- Target DAS28 remission (≤2.6) or low disease activity (≤3.2) within 3–6 months.
- Biologic dose escalation / tapering
- Sustained DAS28 < 2.6 for ≥6 months: consider taper of biologic (RETRO/DRESS trials).
