Complicated SAB
At least 4–6 weeks IV therapy — echocardiography mandatory
All patients with Staph aureus BSI require mandatory ID consultation and TEE.
How to use it
- 1
ALL criteria must be met for uncomplicated. Any single failure = complicated SAB → 6 weeks IV antibiotics.
- 2
TEE mandatory for all SAB. Even 'uncomplicated' SAB: TEE within 5–7 days.
- 3
ID consult mandatory. UK/US guidelines: infectious disease/microbiology review of all SAB within 24h.
When to use it
- Staph aureus bacteraemia (any source)
- Duration decision critical — under-treatment causes relapse/seeding; over-treatment = catheter complications.
- MRSA vs. MSSA
- MRSA: vancomycin/daptomycin for 14–42 days. MSSA: flucloxacillin/nafcillin preferred.
- Line-associated BSI
- Remove source (CVC) + 14 days if uncomplicated criteria met.
