Abstract
We present a rare case of idiopathic adult giant-cell hepatitis (GCH). A 30-year-old female presented with progressive vomiting, jaundice, and weight loss, accompanied by marked hepatocellular injury of unclear cause. A liver biopsy showed multinucleated hepatocytes with severe lobular inflammation, confirming the diagnosis of GCH. Initial clinical and biochemical improvement was achieved with corticosteroids. Subsequent relapses were successfully managed with short courses of 6-mercaptopurine and azathioprine, and the patient has remained clinically and biochemically stable off treatment. This case highlights the variable course of GCH and suggests that sustained disease control may be achievable without chronic immunosuppression.
