Sign concordance of the Lean RECD tick and Level-3 excess3 on pre-VF Holter recordings does not extend to the alarm time
By Johel Padilla.
Abstract
Background. A published analysis of ten Holter recordings that end in spontaneous ventricular fibrillation (VF), from the Sudden Cardiac Death Holter Database (SDDB), contrasted a basal window with an approach window for systemic tau (τs) and for the Level-3 series excess3. The Lean RECD tick ΔT = g(τs)|τs|, built on the gate g of Module CT, had not been compared with excess3 on these recordings. Methods. The analysis used the ten recordings, windows and anchors of the published analysis, W = 101 beats, stride 5, Bandt-Pompe patterns of order 3 with delay 1, and gate parameter kn = 0. For each record, the sign of ΔA, the approach-minus-basal mean of ΔT, was compared with the sign of ΔB = Δexcess3, the zero sets of g and excess3 were checked, and the frozen abs-z alarm detector was applied to both series after a reproduction control. Results. A reproduction control matched the published detector results. The sign of ΔA matched the sign of ΔB in 8/10 records, and records 32 and 45 did not match. The alarm of ΔT matched the alarm of excess3 in 6/10 records. In none of the six double alarms did the lead times agree within one stride-5 step, and the largest difference was −5.15 h in record 51. Conclusion. In these ten SDDB Holter recordings, the sign of ΔA for the Lean tick matches the sign of Δexcess3 in 8/10 records, while the abs-z alarm does not match (6/10) and the lead time does not match in any double alarm.
