高齢者における外傷性腱板断裂の特徴に関するMRI所見の検討

Abstract
本研究の目的は，高齢者の外傷性腱板断裂と非外傷性腱板断裂における術前MRI画像の違いを比較検討することである．65歳以上で腱板全層断裂に対して関節鏡視下腱板修復術を施行した外傷性腱板断裂群（T群）38肩，非外傷性腱板断裂群（D群）45肩を対象とした．外傷性腱板断裂の定義は1．突然発症の外傷，2．発症した外傷の日時，場所が明確，3．交通事故や立位からの転倒転落で腕を伸ばした状態での肩の受傷，4．受傷前は肩関節の症状がないもの，5．腱板構成筋の脂肪変性は軽度のものとした．術前MRI画像にて，上腕骨の骨挫傷性変化，筋腱移行部の浮腫，腱断端のたわみ，Cobra sign，関節内や肩峰下滑液包水腫の有無を評価した．筋腱移行部の浮腫，腱断端のたわみ，関節内や肩峰下滑液包内水腫はT群で有意に多く，特に腱断端のたわみは外傷性腱板断裂を示す独立因子であった．これらのMRI所見は両群の鑑別に有用である可能性が示唆された．
