外側大腿皮神経麻痺を合併した上前腸骨棘裂離骨折に対し手術治療を行った一例

Abstract
外側大腿皮神経麻痺を合併した上前腸骨棘裂離骨折に対し手術療法が奏功した1例を経験した．症例は16歳男性．体育祭のリレー中に右大腿部に轢音を自覚し前医受診した．右上前腸骨棘裂離骨折と診断され保存加療となったが，徐々に右大腿前面から外側にかけての痺れを自覚したため，受傷後1週で当科受診した．翌日に観血的骨接合術を施行した．転位した骨片に連続する筋膜により外側大腿皮神経が圧迫されており，神経剥離を行った後，スクリュー，fiber wire，suture anchorを用いて裂離骨片を母床へ固定した．術後より痺れは消失したが知覚鈍麻は残存し，荷重は術後2週で1/2部分荷重，3週で全荷重とした．知覚鈍麻は徐々に改善し術後10か月で完全に消失した．外側大腿皮神経麻痺を伴う上前腸骨棘裂離骨折に対し，骨接合時に神経剥離を併用することが麻痺改善に有用であると考えられた．
