リバース型人工肩関節置換術後に肺血栓塞栓症を発症した1例

Abstract
【緒言】整形外科術後の合併症として静脈血栓塞栓症（VTE）があるが，ガイドラインでは上肢手術は通常低リスクに分類される．今回血液疾患を合併し,術後にVTEを発症した症例を経験した．【症例】82歳男性．右肩腱板断裂性関節症に対し，リバース型人工関節置換術後9日目に胸痛・SpO2低下が出現し，造影CTで両肺動脈，右下肢静脈に血栓を認め，肺血栓塞栓症（PE）の診断となった．本症例は術前約5年間血小板増多症に対し加療されていたが，術前の血小板数は正常で血栓症も認めなかった．術後精査にて本態性血小板血症(ET)と診断され,VTE発症への関与が疑われた．【考察】血栓性素因やJAK2遺伝子変異陽性のET，血栓症の既往はVTE発症の危険因子で血栓症リスクが高いとされている．本症例はETの疑いはあったものの，血栓症の既往や凝固異常も無い上肢手術であり，術後VTEを予測することは難しい症例であった．
