非転位型大腿骨頚部骨折Pauwels Type 3に対する骨接合術後にインプラント周囲骨折をきたした一例

Abstract
非転位型大腿骨頚部骨折におけるPauwels Type 3に対して角度安定性を有するPrima Hip Screw Side Plate System（以下，Side Plate）が有用であるという報告は多い．今回，Side Plateを用いた骨接合術後にインプラント周囲骨折の一例を経験したので報告する．症例は66歳女性，自宅で転倒し，右股関節痛のため歩行困難となり救急搬送された．単純X線像で骨折は不明であったが，翌日MRIにて大腿骨頚部骨折Garden分類stage 2，Pauwels Type 3，Pauwels角80°を認め，Side Plateを用いて骨接合術を行った．術後2週目で杖歩行自立し自宅退院した．術後4週目に誘因なく右股関節痛が出現したが画像検査では異常所見を認めず経過観察とした．術後9週目に軽微な外傷で転子下の遠位横止めスクリュー周囲でインプラント周囲骨折を生じた．画像を再評価すると同部位に骨膜反応とK-wireを打ち直した骨孔を認め，Pauwels角80°の高い剪断力に加え，骨孔に応力集中しインプラント周囲骨折に至った可能性が示唆された．
