上腕骨遠位骨幹部骨折に対して術前にプレートを切断して使用した治療経験

Abstract
上腕骨遠位骨幹部骨折に対する治療として，当科では上腕骨前方にMIPO法でナローLC-LCP®プレート（DePuySynthes社）を使用している．スクリュー径は5mmを選択し，遠位骨片に必要なスクリュー本数を挿入するため，プレートを可能な限り上腕骨遠位に設置しなければいけないことがある．症例によっては，術前計画でプレートの遠位先端の鉤突窩への干渉や肘関節可動域障害の発生が懸念される．そのような場合，当科では院内施設課に依頼し，術前にプレートの遠位先端を，スクリューホール機構を破綻させることなく切断し，研磨した後にプレートを使用している．プレート切断により6mm先端を短縮することができ，遠位骨片が50mm以上あれば，6ヶ所で皮質骨をスクリュー固定することが可能であった．
