当院における後方アプローチによる両側一期的THAと二期的THAのX線学的脚長差の検討

Abstract
【はじめに】当院での後方アプローチによる人工股関節全置換術（THA）において，両側一期的THAと両側二期的THAのX線学的脚長差を比較検討したので報告する．【対象】2021年4月から2025年9月の間に当院で後方アプローチにて両側一期的あるいは二期的にTHAを受けた患者それぞれ19例38股を対象とした．評価項目は放射線学的脚長差（RLLD），手術時間，術後合併症，術前と術後1年のJOAスコアおよびHHSを調査した．統計学的解析はMann-WhitneyのU検定を用いて検討し，有意水準をp＜0.05とした．【結果】両側一期的THA群では術後平均RLLDは1.8mm，二期的THA群の術後平均RLLDは2.5mmであり，有意差は認めなかった．また手術時間，術後合併症およびJOAスコアとHHSにも両群間に有意差を認めなかった．【結語】両群ともに術後1年の時点では脚長差に有意差はなく，短期ではあるが良好な臨床成績を得た．
