思春期におけるjuvenile Tillaux骨折とTriplane骨折の同時受傷の1例

Abstract
【はじめに】思春期にjuvenile Tillaux骨折とtriplane骨折を両側同時に受傷した極めて稀な1例を経験したので報告する．【症例】15歳男性．道路を走っていたところ左足を段差に引っかけてバランスを崩し，右足で着地しようとした際に右足も捻り受傷．右脛骨にjuvenile Tillaux骨折，左脛骨にtriplane骨折を認め，受傷後3日目に両側同時に中空スクリューを用いて骨接合術を施行した．術後2週から関節可動域訓練を開始．術後4週から起立訓練，術後5週から両松葉杖による歩行訓練を開始し，術後6週から両足とも全荷重を許可．術後3か月でスポーツ復帰も可能となった．【考察】juvenile Tillaux骨折とTriplane骨折は，思春期において骨端線閉鎖前の時期に外旋・底屈力が加わることで発生するとされているが，両側同時に受傷した報告は渉猟しうる限りない．本症例では，稀な両側の足関節骨折であったが慎重なリハビリテーションにより良好な経過が得られた．
