変形性膝関節症と足部足関節疾患を有した症例への下肢アライメント

Abstract
変形性膝関節症に対する高位脛骨骨切り術（OWHTO）により下肢アライメントを外反させた後，同側の進行性扁平足の手術に際し，下肢アライメントの再内反の懸念をきたした症例について報告する．症例は，73歳女性．左膝痛あり，Kellgren-Lawrence分類Ⅲの変形性膝関節症の診断であった．％Mechanical Axis（％MA）は30％と内反変形があり，OWHTOを施行した．術後％MAは78％であった．OWHTO後6年で，同側の進行性扁平足に対して，踵骨内側移動術を施行した．術後％MAは73％とアライメント変化が生じたものの，外反アライメントは維持されていた．踵骨内側移動術前後において，膝JOAスコアは90点と不変であり経過良好であった．進行性扁平足に対する術後のアライメント変化を考えると，OWHTO術前に進行性扁平足について確認することが望ましいことが示唆された．
