Abstract
大腿四頭筋腱断裂は膝伸展機構損傷の中でも比較的稀な外傷であり，加齢に伴う腱変性，糖尿病，関節リウマチなどの基礎疾患を有する中高年男性に多い．今回，当院で手術加療を行った4例4膝（新鮮例3例，陳旧例1例）について治療成績を検討した．全例男性で，平均年齢63歳，平均観察期間8ヵ月であった．全例にpull-out法を施行し，最終観察時の平均可動域は伸展－2.5°，屈曲130°であった．陳旧例1例で再断裂および感染を認め，再手術を要した．陳旧例に対しては再建術・補強術の有用性が報告されているが，本症例では術中に腱短縮を認めず，糖尿病による感染リスクを考慮してpull-out法を選択した．受傷時期，基礎疾患，活動性などを総合的に評価し，症例ごとに適切な術式選択が必要であると考えられた．
