尺骨非定型骨折に対し手術および補助療法が奏功した一例

Abstract
【緒言】近年，ビスフォスフォネート（BP）製剤長期使用後の尺骨非定型骨折の報告が散見されるが，いまだ確立された治療戦略はない．尺骨非定型骨折に対し手術および補助療法が奏功した一例を経験したので報告する．【症例】84歳女性．骨粗鬆症に対しBP製剤を約7年内服．明らかな外傷なく左肘痛があり前医受診し，左尺骨非定型骨折と診断されテリパラチド（TPD）を導入のうえ保存的治療が開始された．その6か月後にベッド上で左手をついて受傷し，当院へ救急搬送された．左尺骨近位骨幹部に骨膜肥厚を伴った完全横骨折を認めた．受傷後11日目にdouble plateによる骨接合術を施行した．術後よりTPDを継続し，LIPUSも併用した．術後6か月と比較的早期に骨癒合が得られた．【結語】尺骨非定型骨折に対するdouble plateによる強固な内固定とTPD，LIPUSの併用は有効であった．
