外反膝に対して脚長差を考慮してdlo（mcwdfo, lowhto）を施行した一症例

Abstract
【要旨】外側変形性膝関節症は大腿骨遠位骨切り術の適応となるが，高度外反の場合脛骨側にも変形が生じ，double level osteotomy（DLO）が必要となることがある．しかし適応症例は少なく報告も少ない．今回，脚長差を考慮して大腿骨内側閉鎖式骨切り術（MCWDFO），脛骨外側開大式骨切り術（LOWHTO）を施行した症例を経験したので報告する．48歳女性．20代から左膝痛が出現し44歳時に近医で関節鏡下外側半月板切除術が施行された．以降保存療法が継続されたが左膝痛の増悪認め当院を受診した．立位下全長X線にて％MA 108％，mLDFA 81度，mMPTA 91度と著明な外反変形を認め，またSMDで4mmの脚長差を認めた．患者は運動継続希望があり，術後の脚長差を考慮してMCWDFO，LOWHTOを施行した．術後1年で抜釘し，％MA 24％，mLDFA 93度，mMPTA 87度，脚長差は術前と変わらず4mmであり軽運動可能となった．高度外反膝に対してのDLOは複数の術式が考えられるが脚長差を考慮するならばMCWDFO，LOWHTOは有用な選択肢となりうる．
