By Dennis Simon, Rennis Davis, Partha Sarathi Ghosh and 1 others.
Abstract
Chest radiography demonstrated bilateral mid-and lowerzone infiltrates with pleural effusions (Fig. 1).Electrocardiography showed an S1Q3T3 pattern suggestive of acute PE.Pleural fluid analysis revealed a lymphocyte-predominant exudate with sterile cultures.Arterial blood gas analysis showed type I respiratory failure.Laboratory investigations revealed hemoglobin 9.2 gm/dL, platelet count 90,000/mm 3 , reticulocyte count 4.33%, lactate dehydrogenase (LDH) 407 U/L, and D-dimer >10,000 ng/mL.Transthoracic echocardiography demonstrated severe pulmonary hypertension with mild right atrial and right ventricular dilatation.
