Abstract
A BSTRACT Background: The popliteus is a small yet crucial muscle in the posterolateral corner of the knee, responsible for initiating knee flexion and stabilising against external tibial rotation and varus forces. Its deep anatomical location often leads to under-recognition, particularly in noncontact overuse injuries. Case History: This case series reports on three elite male race walkers who developed chronic posterior knee pain during downhill walking and terminal knee extension, especially under high training loads. All athletes had normal imaging findings (MRI, ultrasound), but clinical examination revealed posterolateral tenderness and positive provocative tests specific to the popliteus. Gait analysis demonstrated contralateral pelvic drop and trunk lean, suggesting kinetic chain inefficiencies.
