Abstract
Health services in Correctional Institutions (Lapas) are human rights guaranteed by law, yet their implementation is often hindered by overcrowding and limited resources, particularly in managing infectious diseases such as Tuberculosis (TB). This study aims to analyze strategies for improving health services for inmates suffering from TB through a treatment assistance program at the Class IIB Purwokerto Narcotics Correctional Institution and to identify supporting and inhibiting factors. A descriptive qualitative research approach was employed. Data were collected through in-depth interviews, passive participant observation, and documentation studies. The findings were analyzed using the five SERVQUAL dimensions (Tangibles, Reliability, Responsiveness, Assurance, and Empathy) and the Directly Observed Treatment Short-course (DOTS) framework. Results indicate that the treatment assistance program successfully reduced the number of TB cases from 12 in 2025 to 5 in 2026. However, program execution still faces challenges, including a shortage of medical staff, inadequate isolation and ventilation facilities due to the heritage building structure, manual medical record-keeping, and social stigma among inmates. Strengthening strategies involve enhancing the competence of Treatment Observers (PMO), cross-sector collaboration with the Banyumas Health Office, digitizing health information systems, and providing psychosocial support.
