Evidence-Informed Consensus Treatment Algorithm for Orbital Floor Fractures and the Management of Complications: An International Modified Delphi Study
By Michał Gontarz and Kamil Nelke.
Abstract
International, evidence-informed modified Delphi study developing a consensus-based, stepwise treatment algorithm for orbital floor fractures and a structured pathway for the management of their complications. A systematic review generates single-concept statements across eight clinical domains (diagnostic work-up, indication for surgery, timing, surgical approach, reconstructive material in adults and children, intra-operative adjuncts, post-operative care and follow-up, complications). A multidisciplinary international panel (target 40–60 experts in craniomaxillofacial surgery, oculoplastic surgery and ophthalmology) rates statements on a 9-point scale over three anonymous online rounds with controlled feedback, against a priori consensus (≥75% rating 7–9 and <15% rating 1–3) and stability criteria. A virtual consensus meeting assembles endorsed statements into the final algorithm. Funded by IBRA (REC Grant 2026, CMF). Reported per CREDES.
