Abstract
Abstract ICLA04 - The Labral Repair Blueprint: Mastering Anchorage, Suture Loops, and Everyday Success, Auditorium B (BMCC Level 1), October 8, 2026, 10:30 - 12:30 Background Femoroacetabular impingement (FAI) is a common cause of hip pain in young athletes, particularly in gymnastics, where repetitive hip flexion, extreme range of motion, and supraphysiologic flexibility increase the risk of intra-articular pathology. While hip arthroscopy with labral repair has demonstrated favorable outcomes in athletic populations, outcomes in elite gymnasts remain poorly characterized despite the unique biomechanical demands and high expectations for return to sport. Purpose To evaluate minimum 2-year patient-reported outcomes following hip arthroscopy with labral repair for FAI in elite gymnasts. Study Design: Case series; Level of evidence, 4. Methods A retrospective review was performed of elite gymnasts who underwent hip arthroscopy with labral repair for FAI by a single high-volume hip preservation surgeon. Patients with a minimum 2-year follow-up were included. Patient-reported outcomes included mHHS, HOS-ADL, HOS-Sport, SF-12 PCS, WOMAC, satisfaction, and Tegner activity level. Results: 34 elite gymnasts (47 hips) were included. Minimum 2-year outcome data were available for 29 hips from 19 patients, with 18 hips from 15 patients lacking complete follow-up. At follow-up, patients demonstrated high function, with mean mHHS of 81.0 ± 22.1 (median 93), HOS-ADL of 87.7 (median 96), and HOS-Sport was 79.0 (median 94). Mean SF-12 PCS was 50.9 and WOMAC was 13.4. Patient satisfaction averaged 8.1/10, and mean Tegner activity level was 5.2, consistent with return to high-level athletic activity. Conclusion Elite gymnasts demonstrated favorable clinical outcomes and high functional recovery at minimum 2-year follow-up after hip labral repair. These findings support hip arthroscopy as a reliable treatment for FAI in this high-demand athletic population.
