Refined classification and surgical techniques for alar base reduction in rhinoplasty: A stepwise approach for optimal aesthetic outcomes
By Ahmet Alp and İsa Özbay.
Abstract
Background Alar base surgery is a key component of rhinoplasty for achieving balanced nasal proportions and harmony with facial features. Variations in nasal base morphology affect both aesthetic and functional outcomes. Methods We present a practical classification framework for alar base deformities and describe a stepwise surgical approach tailored to patient-specific anatomical and ethnic characteristics. Surgical strategies for wide alar base, isolated flare, combined deformities, thick alar base, and thick lobule are outlined. Practical intraoperative pearls and tissue-sparing techniques are emphasized. Results Individualized incision design and resection pattern allowed precise correction of nasal base asymmetry while preserving natural nostril contour. Postoperative outcomes demonstrated improved nasal aesthetics with minimal scarring. Conclusion A structured approach to alar base surgery, guided by a practical classification system and patient-specific planning, optimizes aesthetic results and maintains functional integrity. Level of evidence V (Expert Opinion).
