Abstract
ABSTRACT A 44‐year‐old man developed otogenic sigmoid sinus thrombosis with cerebellar abscess, hydrocephalus, and recurrent posterior fossa infection. Staged neurosurgical drainage and canal‐wall‐down mastoidectomy achieved source control. Cultures were negative. Anticoagulant exposure was perioperative/early postoperative. Despite persistent venous non‐opacification, he was discharged improved, afebrile, without neurological deficit, and with remitted otoliquorrhea.
