Low risk (<4) — mechanical prophylaxis; reassess daily
How to use it
- 1
11 weighted risk factors. Active cancer, prior VTE, immobility, thrombophilia: 3 pts each.
- 2
Padua ≥4 = high risk. VTE rate ~11% without prophylaxis vs. ~2.2% with LMWH.
- 3
Bleeding risk assessment also needed. High VTE risk + high bleeding risk: mechanical prophylaxis (IPC) preferred.
When to use it
- Medical inpatient admission
- NICE NG89: assess VTE risk (Padua or IMPROVE) on admission for all medical patients.
- Reassessment if condition changes
- New immobility, cancer diagnosis, or procedure: re-score and review prophylaxis.
- Discharge planning
- Extended prophylaxis (LMWH 14–28 days) for high-risk patients post-cancer surgery.
