Abstract
Background
Periventricular high-grade gliomas (PVGs) are associated with high approach-related risk due to their deep location, ventricular wall involvement, and proximity to subcortical pathways. While extent of resection remains a critical prognostic factor, approach-related morbidity often limits the cytoreduction. Currently, no practical topographic classification exists to guide selection of safe operative corridors in PVGs. This study proposes a ventricular-wall topographic classification for PVGs, describes how it informs corridor selection, and reports early extent of resection and functional outcomes in a pilot single-center series.
