Abstract
Acute ischemic stroke involving the M2 segment of the middle cerebral artery can produce substantial neurological deficits. Recent AHA/ASA recommendations support consideration of endovascular thrombectomy in carefully selected patients with proximal M2 occlusions, while intravenous thrombolysis remains a mainstay of reperfusion therapy for eligible individuals. Because thrombolytic therapy is restricted by time-dependent efficacy and eligibility criteria, mechanical thrombectomy may provide an alternative means of reperfusion. Nevertheless, intervention for medium-vessel occlusions poses technical challenges and carries a risk of vessel injury and intracranial hemorrhage. Partial deployment of the stent retriever may reduce mechanical stress on the distal vessel wall while preserving thrombus capture. This case illustrates successful reperfusion and favorable neurological recovery following use of this technique.
