Abstract
We report the case of a 61-year-old man with advanced HIV who presented with progressive dyspnea, productive cough, and violaceous cutaneous lesions; computed tomography (CT) of the chest demonstrated bilateral, asymmetric, flame-shaped peri-bronchovascular opacities suggestive of pulmonary Kaposi sarcoma (KS). An initial CT-guided core needle biopsy of the left lung was suspicious for adenocarcinoma but indeterminate on scant tissue. Repeat CT-guided core needle biopsy of the left lower lobe and lingula confirmed KS by human herpesvirus 8 latent nuclear antigen-1 positivity. This case highlights the value of repeat tissue sampling when pathology results are discordant with strong clinical and radiographic suspicion.
