Abstract
Slides of a talk given at the 2026 CCAIM Clinical Summer School, Cambridge, 19 September 2026. The talk argues that the question a patient asks — what would happen to me with and without treatment — meets five walls: three about the probability (the comparison is never observed, no cohort contains his presentation, his history cannot be recovered) and two about the people who use it (his values and the clinician's own judgement). It presents IQ-MDM and a mechanistic counterfactual engine for early prostate cancer, including calibration against published data and two validation tests reported as they came out. Slides are as presented; results obtained after the talk are not included.
