開放性膝蓋腱近位付着部断裂に対して人工靱帯を併用し靱帯修復を行なった1例

Abstract
症例は64歳女性，草刈り機のブレードが右膝前面に当たり受傷，13 cmの膝関節内に達する開放創で，膝蓋腱の膝蓋骨付着部での完全断裂と外顆の骨欠損を認めた．洗浄と一時的な腱縫合を行い抗菌薬全身投与，受傷後7日目に膝蓋腱のpull-out縫合に人工靱帯を併用し靱帯修復術を行なった．術中は屈曲90°で離開がないことを確認した．2週間の外固定後に荷重と可動域訓練を開始し，感染と再断裂なく経過した．術後2ヶ月で屈曲50°と制限あり関節鏡併用下関節授動術を行なった．関節内の瘢痕組織の切離と徒手的な関節授動術を行い，術中は屈曲120°まで可能となった．術後11ヶ月の最終観察時，extention lagなく屈曲135°，独歩可能であった．断端の挫滅や腱付着部での損傷等による膝蓋腱の癒合不全のリスクや，関節包の広範な損傷による術後癒着での可動域制限残存のリスクが高い症例であったが，pull-out縫合と人工靱帯による補強で，早期の関節授動術に耐えうる強固な修復が可能であった．
