橈骨粗面の骨棘による近位橈尺骨インピンジメント症候群の2例

Abstract
【背景】橈骨粗面に生じた骨棘変化が原因となり，橈尺骨間で疼痛を生じた橈尺骨インピンジメント症候群の2例を経験した．本疾患の文献報告は非常に少ない．発症機序や画像所見，手術方法について検討した．【症例】症例1，81歳女性．誘因なく右肘の回内外時にクリックとともに肘痛が出現した．CTで橈骨粗面に骨棘を確認し，MRIで遠位上腕二頭筋腱（distal biceps tendon：DBT）付着部に炎症を認めた．手術では，骨棘と回外筋稜がインピンジしており，DBT長頭が挟まっていた．骨棘切除およびアンカーを用いたDBT再縫着術を行った．症例2，78歳女性．転倒後2週間して右肘内側に疼痛を訴えて来院した．CTで橈骨粗面に骨棘を認め，DBTを温存して骨棘のみを切除した．【結論】両症例も術後の疼痛は速やかに改善した．本疾患は，骨棘が回外筋稜とインピンジし，DBT長頭が摩擦して生じる．橈骨粗面の骨棘切除術が有効であり，腱損傷を伴う場合には腱再縫着の併用も有用と考えられた．
