同側の大腿骨頚部骨折を合併した寛骨臼両柱骨折に対し寛骨臼骨接合術と人工骨頭挿入術を併用した1例

Abstract
寛骨臼骨折と同側大腿骨頚部骨折の合併は稀であり，多くは高エネルギー外傷による．単独損傷であれば転位型の大腿骨頚部骨折には人工骨頭置換術（BHA）や人工股関節置換術（THA），寛骨臼骨折には骨折観血的手術（ORIF）が行われるが，同時受傷例に対しては急性期THAの有用性も報告されており，標準的治療は定まっていない．症例は65歳男性．約4 m転落し，寛骨臼両柱骨折と同側の大腿骨頚部骨折（Garden分類stage IV）を受傷．受傷5日に前方アプローチで前柱のORIF，8日に後方アプローチで後柱のORIFとBHAを施行した．周術期合併症は認めず，術後1年でJOAスコアは89点でADLは一本杖歩行．術後2年でも関節裂隙の狭小化やインプラントのゆるみを認めず，良好な経過を維持している．同側の大腿骨頚部骨折を合併した寛骨臼骨折に対し，ORIFとBHAの併用は有用な治療法の1つとなり得る．
