外傷性恥骨結合離開に動脈損傷を合併した1例

Abstract
【背景】骨折を合併しない恥骨結合離開（AO分類 61B1.2，Young-Burgess分類 APC-1）は骨盤輪損傷に分類されているが，これまで動脈損傷を合併したとされる報告は認めない．今回，骨折を有しない外傷性恥骨結合離開に動脈損傷を合併した1例を経験したので報告する．【臨床経過】58歳 男性．2輪車運転中に乗用車に追突し，受傷され前医へ救急搬送された．左恥骨部直上に圧痛を認め，レントゲン上明らかな骨折はないものの軽度の恥骨結合離開を認めた．CT上も明らかな骨折は認めず同日，安静目的で入院となった．入院翌日，下腹部痛が急激に増悪し，造影CTにて内腸骨動脈の恥骨枝から活動性の出血を認めた．同日に当院へIVR目的で転院となった．【考察】本症例では骨折はないものの，高エネルギー受傷起点による前後方向への直達外力によって動脈損傷が生じたものと推察する．高エネルギー受傷起点の場合，血管損傷を念頭に造影CT撮像や血液検査によるフォローが必要と考える．
