Abstract
【目的】Open wedge distal tibial tuberosity osteotomy（OWDTO）はPF関節障害が少ないが，骨癒合遷延が懸念されるため，当院では粗面部から2本のAPスクリューを挿入していた．粗面部の骨切りと皮膚切開を短くするために，2024年よりAPスクリューを1本挿入するinterlocking OWDTO（IL-DTO）を行なっている．今回はその短期成績を検討した．【対象と方法】対象は2024年1月から2024年7月までにIL-DTOを施行し術後1年経過し抜釘を行った14例15膝とした．術前，術後3週，3ヶ月，6ヶ月，9ヶ月，12ヶ月の％MA，mMPTA，TPSを評価した．また当院で施行したOWDTO 15膝と骨癒合時期の比較検討を行った．臨床評価としてKOOS，Lysholm knee score，合併症について調査した．【結果】骨癒合時期はOWDTOと比較して有意差はなかった．ヒンジ骨折を3膝，フランジ骨折を1膝で認めたが，骨癒合遷延例は1例もなく，術後1年までに全例骨癒合が得られた．【結語】IL-DTOの短期成績は良好であった．骨切り部で粗面部のフランジが安定するため，骨癒合に有利な可能性がある．
