A surgical case of small-sized lung adenocarcinoma combined with pulmonary aspergillosis

Abstract
一般に肺アスペルギルス症を合併した肺癌は進行肺癌が多く，小型肺癌にアスペルギルスが合併した症例は極めて稀である．今回我々は小型肺癌にアスペルギルス症を合併した1例を経験したため報告する．症例は76歳男性，2023年12月から指摘されていた右上葉の結節が経時的に増大し当科に紹介された．CTでは右上葉S1末梢に胸膜陥入を伴い辺縁不整な部分充実型の結節影を認めた．診断と治療の目的で手術を行う方針にした．二孔式のロボット支援下右上葉切除術を施行した．病理組織所見では腺癌内線維化の内部にアスペルギルスの菌塊を認め，肺癌の病期はpT1aN0M0-StageIA1であった．現在まで術後14ヵ月経過したがアスペルギルス症の再燃や肺癌の再発は認めていない．
