A case of solid pseudopapillary neoplasm arising in the lung

Abstract
症例は36歳の女性で検診にて胸部異常陰影を指摘された．前医を受診し，胸部CTで右肺上葉に3 cm大の辺縁明瞭な腫瘤性病変を認め，当科へ紹介となった．気管支鏡検査で核腫大・核小体明瞭な，軽度異型性を示す腺細胞の集塊が観察された．画像所見の形態から過誤腫が疑われたが，過去の検診で指摘されなかったことから低悪性度腫瘍の可能性も否めず，手術加療の方針とした．術中迅速病理検査で悪性所見はなく，胸腔鏡下右肺部分切除術を施行した．最終病理検査ではSolid pseudopapillary neoplasm（以下，SPNと略記）と診断された．他臓器の検索として膵臓には造影CTや超音波内視鏡で精査し，卵巣には超音波・造影MRIで精査したが，腫瘤性病変を認めなかった．非常に稀な，肺にSPNが発生した症例を認めたために，若干の文献的考察を含めて報告する．
