Intercostal artery pseudoaneurysm diagnosed after traumatic hemopneumothorax caused by glass fragments: A case report

Abstract
症例は80代前半，男性．1週間前に自宅で転倒した際に右背部にガラス片が突き刺さり，近医に搬送された．創部の処置のみで経過観察となったが，受傷6日目に左胸痛を認め，近医を受診し，左気胸の診断で当科に紹介となった．左血気胸に加え，左背部には異物を認めた．胸腔ドレーンを挿入し，血性排液を認めたため，異物除去と止血術のため緊急手術を施行した．術後5日目に造影CTで左第7肋間動脈の仮性動脈瘤を認めた．N-butyl-2-cyanoacrylateによる肋間動脈塞栓術を行った．肋間動脈の仮性動脈瘤の文献はいくつか散見されるが，比較的稀とされている．仮性動脈瘤の原因として外傷や医原性などがあるが，原因は不明であった．今回，我々は外傷性血気胸の術後に肋間動脈の仮性動脈瘤を診断した症例を経験したため，文献的考察を加えて報告する．
