Abstract / Summary
Mucous membrane pemphigoid (MMP) is a chronic autoimmune subepithelial blistering disorder with significant morbidity, especially in cases with ocular involvement, often requiring prolonged systemic immunosuppression. We report a 71-year-old male with refractory MMP involving oral and ocular mucosa, who showed inadequate response to daily systemic corticosteroids and mycophenolate mofetil and declined biologic therapy. He was treated with dexamethasone–cyclophosphamide pulse therapy, leading to marked clinical improvement; cyclophosphamide was later replaced with azathioprine due to leukopenia, following which complete remission was achieved and sustained. This case highlights pulse therapy as an effective, practical alternative in refractory MMP, particularly when conventional or biologic therapies are not feasible.
